The Creative Smiles Savings Program is an annual savings program that has been developed to deliver quality dental services to you and your family from Creative Smiles at fees that make sense for today's economy. This program will ensure that all dental needs will be met with individualized care.
This is not an insurance plan, no claims are filed, and no payments are made to any other healthcare providers. Creative Smiles is not a licensed insurer, health maintenance organization or other underwriters of health services.
The program may not be combined with any other offers, discounts, insurance, discount plans or advertisements. The savings offered are valid only in the office.
WHAT IS the Annual Savings Program and what Does it Include
One year of dental care for only $179 (one person) includes the following:
- Two Oral Exams
- Periodontal Evaluations
- Oral Cancer Screenings
- Digital Dental Mouth X-Rays
- 20% OFF Periodontal Maintenance Cleanings
- 20% OFF All Other Dental Services and Treatments
WHEN Will Benefits Begin?
Benefits will begin immediately and are effective for 12 months. You have the right to cancel at anytime by submitting a written request. The refund will be prorated reimbursement of the annual fee paid less a $40 processing fee.
